A Case of Multiple Symmetric Lipomatosis

Abstract
ackground : Multiple symmetric lipomatosis (MSL) is a rare disease characterized by the formation of symmetrically located nonencapsulated nontender lipomatomas at the neck, shoulder, mediastinum and occasionally in abdomen, buttocks.
That
is
often associated with
excessive alcohol intake and usually presented no clinical symptoms but infrequently
expressed by muscle weakness, dyspnea due to airway compression. The pathogenesis of
MSL is obscure, but defect of adrenergic stimulated lipolysis is strongly suggested.
Results : We experienced 4 cases of MSL which presented with lipomatas at the neck,
abdomen and back. Among these cases, 1st case patient showed defect of adrenergic
stimulated lipolysis. Observation of free fatty acid level after the infusion of noradrenaline
reveals that plasma FFA levels were lower than those of control person. Follows-up of 2 to
3 months after use of theophylline 300 mg p.o. daily as a therapeutic trial, the size of lipoma
was decreased significantly in all cases.
Conclusion : We report 4 cases of MSL with the review of literatures and re-emphasize
that the phosphodiesterase inhibitor is useful as a medical therapeutic agent in clinical trial
of MSL
